LLM23 summary form guidelines

The review discussion should be confidential and honest. It is also designed to be structured, positive, affirming and challenging.

You are expected to complete the summary form with the Reviewer at the end of the review discussion. In a similar way to the self assessment preparation form you may complete this in a way that is helpful to you and you should not feel constrained to fill in every box.

However as part of the discussion you and your Reviewer will identify and agree objectives and actions and these should be completed in the summary. The objectives will include areas you wish to prioritise in your ministry (relating to both your own development and the needs of your ministry setting) and the timescales may be short, medium or long term to cover the period until your next MDR. You are also invited to consider the resources or help you need to achieve the objectives.

You are not expected to have more than four or five objectives in total but they should be SMART – Specific, Measurable, Achievable, Realistic and Time bound.
