LLM24 reference form guidelines

As LLMs do not minister in isolation it is highly beneficial to have feedback about their ministry and recommendations on areas that may be developed further. It is essential that your incumbent completes and returns the feedback form and also shares it with you well in advance of your MDR.

Your incumbent can complete the form in a way that is most helpful to him or her, to reflect on your ministry in a directed way and to highlight those areas that are most relevant.

In addition you may choose to ask others to provide feedback for your review and this form may be used for that purpose too. In these circumstances you should be very clear about whether the form is to be returned to you so the feedback is given to you directly, to your consultant or to the MDR administrator to pass to your Consultant.
